– Also presents with multiple comorbid conditions including HIV and hepatitis C

– Patient received all medications necessary to control his hemophilia and comorbidities from the same local specialty pharmacy for 16 years

– As of January 2011, changes to his insurance plan required the use of different specialty pharmacies for each of his chronic conditions

• Use of multiple pharmacy providers has fragmented the care received by the patient
• Inconsistencies in care delivery have hindered his ability to effectively manage his hemophilia and his concomitant chronic comorbid conditions
