• Patient received factor shipment for dental procedure
  

  –	Home health agency administered factor in the home setting prior to each dental procedure
  –	Nurse case manager coordinated infusion and communication plan with HTC to address scheduled infusions and ongoing treatment of any abnormal bleeding
  –	Nurse case manager continued follow-up at weekly intervals for 1 month until both procedures were completed and healing was achieved
